You may pick up an application at the Office of the City Clerk, or download the form from the U.S. Department of State’s website: Passport Application

PASSPORT RENEWALS

For most renewals, you do not need to appear in person.

You can renew by mail if:

  • Your most recent passport is available to submit and it is not damaged;
  • You received the passport within the past 15 years;
  • You were over age 16 when it was issued;
  • You still have the same name, or can legally document your name change.

If your passport has been altered or damaged, you cannot apply by mail. You must apply in person.

Please bring all necessary documentation with you to your appointment, as follows:

Completed, UNSIGNED application – the application must be signed in front of the passport acceptance agent.

EVIDENCE OF U.S. CITIZENSHIP

All documents must be originals

  • Original or certified copy of birth certificate, OR
  • Previous U.S. passport*, OR
  • Consular Report of Birth Abroad or Certification of Birth OR
  • Naturalization Certificate, OR
  • Certificate of Citizenship

*Note: For Minors under Age 16 you must establish proof of citizenship AND proof of relationship.  Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian(s).  See the State's website for further information on proof of relationship: http://www.travel.state.gov/passport/get/minors/minors_834.html

IDENTIFICATION

Submit a Photocopy showing the front and the back of the original Identification Document Presented. Copy must be one-sided on plain white, 8 1/2"x11" standard paper

  • Current, valid driver’s license
  • Government ID: city, state or federal
  • Military ID: military and dependents

PASSPORT PHOTOS

  • Photo Service is available at the City Clerk’s office, OR
  • Bring 2 identical color passport photos, 2x2 inches in size, taken within the past 6 months (please check the U.S. Department of State website for further requirements)

* Expedited Service (is now available for both the Passport Book and Passport Card)
If you choose expedited service, you can expect to receive your passport in about 3 weeks.  In order to save on postal delivery time, you may also request **overnight delivery to ($18.30) and from ($14.96) the passport agency. 

NOTES

  • Both parents must appear and present evidence that they are the child’s parents or legal guardians for a minor under 16 years of age.
  • ALL applicants must be present.
  • Please allow about 6 weeks for regular application processing.
  • Tracking the status of your application:
    You can check the status of your application online at http://www.travel.state.gov Currently, it is taking 7-10 days for applications to be tracked.
  • Passport Card valid for land and sea crossings between the U.S. and Mexico, Canada, Bermuda and the Caribbean only.
